Forms Available in re:Members AMS

All non-itemized custom forms in re:Members AMS can be edited under the Customizations App > Custom Forms.

Once you've narrowed down the type of record on which you want to collect data (Individual, Organizations, Committees, or Chapters) you'll need to locate the necessary custom form.

Based on the name of the form, you can generally infer the area in which the form will appear, and the type of data it should collect.

For example, the Account.Signup.IndividualInfo form can be used to collect data about individuals during signup. The Award.AwardNomination form can be used to collect data about Individuals during the entry of a nominee for an award.

Once you've located the desired custom form, click Customize to begin editing it. New fields can be created during the process of creating the custom form, or separately and selected when editing a form.

Note: Not included in this list are itemized custom forms. If the name of the form ends in “.Itemize” this is an itemized form. While itemized forms are included in the Customizations App > Custom Forms list, they should not be edited from this location. Itemized forms must be customized via the product record.

  • Account.Signup.IndividualInfo: Appears when a new user navigates to the re:Members AMS login page and clicks “Create Account.” This form will display during the account creation process.
  • Award.AwardNomination: Appears in the award nomination process, on web pages using the "Award Nominations for Public" content item and on the Nominee Record.
  • Chapter.Demographics.Edit: Configurable to collect additional information in the Chapter Record.
  • Courses.Wizard.IndividualInfo: Appears during the signup process for certification courses.
  • Crm.Committee.Roster: Appears on web pages using the Committee Roster web content item.
  • Crm.Individual.Demographics.Add: Appears for staff users at the bottom of the “Add New Individual” form, allowing staff to complete custom fields during the add individual process.
  • Crm.Individual.Demographics.Edit: Appears on the Individual Record > Account tab, at the bottom of the Personal Info menu page.
  • Crm.Organization.Demographics.Add: Appears for staff at the bottom of the “Add Organization” form.
  • Crm.Organization.Demographics.Edit: Appears in the Organization Record > Account tab > Organization Info menu page.
  • Crm.Organization.PACSolicitationPermissionHeader: Appears in the Organization Record, Account tab, PAC menu page.
  • Crm.Organization.PACSolicitationPermissionFooter: Appears in the Organization Record, Account tab, PAC menu page.
  • Crm.Organization.Roster: Appears on web pages using the "My Organization Custom Form" content item, as part of the Organization Roster setup.
  • Courses.Wizard.IndividualInfo: Appears in the Certification Course Purchase Wizard.
  • Events.Wizard.IndividualInfo: Appears in the Event Registration Wizard, when completing an individual registration.
  • Events.Wizard.OrganizationInfo: Appears in the Event Registration Wizard, when completing a group registration for an organization.
  • Exhibits.Application.OrganizationInfo: Appears in the Booth Application Wizard and collects data on the organization applying for the booth.
  • Exhibits.RegistrationWizard.IndividualInfo: Appears in the Exhibit Registration Wizard, for individuals.
  • Exhibits.RegistrationWizard.OrganizationInfo: Appears on the Exhibit Registration Wizard, for organizations.
  • Membership.Wizard.IndividualInfo: Appears in the Membership Purchase Wizard when purchasing a membership for an individual.
  • Membership.Wizard.OrganizationInfo: Appears in the Membership Purchase Wizard when purchasing a membership for an organization.

  • Products.PurchaseWizard.IndividualInfo: Appears in the Product Purchase Wizard for individuals, during checkout.
  • Products.PurchaseWizard.OrganizationInfo: Appears in the Product Purchase Wizard for organizations during checkout.
  • Products.Publication.PurchaseWizard.IndividualInfo: Appears in the Publication Purchase Wizard for individual, during checkout.
